- Fill out the Medical-Data Form - This is where we collect your medical information (medications, drug allergies, conditions, etc.). The form is completely encrypted for security.
- Print your MedScope ID Card - Your wallet-sized ID Card provides brief instructions to access your medical information online! The ID Card can be printed directly from our website.
- Carry your ID Card with you - By presenting your MedScope ID Card, you will allow physicians to access your medical information within seconds from any computer with an internet connection!
Changes to your medical data are easily done as often as you like. The form will even display the date of your last update!
